House Call Doctor in West Valley City, Utah

West Valley City is Utah's second-largest city, home to many families, cultures, and languages. For a lot of older residents here, the biggest barrier to care is getting there: one car in the family, adult children at work, or a long bus ride. Dvora Healthcare's physician and nurse practitioner make house calls to patients at home and in assisted living, memory care, and independent living communities.

Every Dvora appointment happens in the patient's own living space, never in a clinic. We are a physician practice, not a home health agency, a hospice, or a personal care service.

Reach us at 801-407-9409 or send a West Valley visit request.

Throughout West Valley City

We visit patients in Granger and Hunter, near Fairbourne Station and Valley Fair Mall, around the Utah Cultural Celebration Center and the Maverik Center, and in the western neighborhoods toward the Oquirrhs.

West Valley City Services

Mobile Primary Care

Routine check-ups, sudden illnesses, prescriptions, and specialist referrals can all be handled at home, so nobody has to leave a shift early to drive Grandma across Bangerter Highway. Primary care details.

Palliative Care

Families in Granger and Hunter caring for someone with a serious illness can get palliative care at home: help with pain and other symptoms, comfort, and planning conversations. Hospice is a separate service we do not provide. Palliative care details.

Chronic Care Management

Diabetes, high blood pressure, heart disease, and lung disease are common long-term conditions, and our staff calls between house calls to keep medications and appointments moving. Chronic care details.

Transitional Care

Holy Cross Hospital – Jordan Valley West sits right inside West Valley City, and many residents are discharged from there. After that stay, or one anywhere else, we can visit to go over the discharge plan and medication changes. Transitional care details.

West Valley City House Call Questions

Do you provide house calls in West Valley City?

Yes. From Granger and Hunter to the newer neighborhoods west of 5600 West, we see patients in private homes and in assisted living, memory care, and independent living communities.

Our family shares one car. Can you come to our home?

Yes. The physician or nurse practitioner drives to you, so the family car can stay with whoever needs it for work.

Can you help after a stay at Holy Cross Hospital – Jordan Valley West?

Yes. Soon after discharge we can come to the house, read through the hospital's instructions with you, check every medication, and set up next appointments.

Do you provide personal care or home health aides?

No. Our visits are medical appointments with a physician or nurse practitioner. Bathing, dressing, home health therapy, and hospice come from other kinds of agencies.

Will insurance cover a visit?

That depends on the plan you carry. We check benefits during intake and explain likely copays or deductibles before the first appointment.

Where is your office?

Our administrative office is in Sandy, and patients are never seen there. Every visit happens wherever the patient lives.
