Dvora Healthcare clinician visiting an older patient at home in Sandy, Utah

House Call Doctor in Sandy, Utah

Getting to a doctor's office is not always easy. For many older adults in Sandy, a trip to the clinic means arranging a ride, a long wait, and a tiring day. Dvora Healthcare brings the visit to you. Our physician and nurse practitioner see patients where they live: at home, in assisted living, in memory care, or in independent living.

We are a mobile physician group. We do not offer office visits, and we are not a home health, hospice, or personal care agency. We are your medical providers, and we come to you.

Care Close to Home, All Across Sandy

Sandy stretches from the Jordan River on the west to the foothills near Little Cottonwood Canyon on the east. We visit families in Historic Sandy and the neighborhoods near the Civic Center, in the middle of the city around Dimple Dell Regional Park, and up the east hillside in areas like Alta Canyon, Willow Creek, and Bell Canyon.

Whether you live in a family home off 9400 South or in a senior living community near State Street, the care is the same: unhurried visits, a provider who knows your history, and a plan you and your family can follow. Our administrative office is in Sandy, so our team knows the area well. All patient visits happen in your home or community, not at our office.

Our Services in Sandy

Mobile Primary Care

Regular check-ups, new symptoms, medication reviews, and ongoing care, all in your own living room. Our providers take time to listen and to work with your specialists and family. Primary care details.

Palliative Care

Palliative care is extra support for people living with a serious illness. It focuses on comfort, symptom relief, and making sure your care matches what matters most to you. It can be given alongside other treatment, and it is not the same as hospice. Palliative care details.

Chronic Care Management

Living with more than one long-term condition, like diabetes, heart failure, COPD, or high blood pressure, takes steady attention. Between visits, our team helps keep track of medications, follow-ups, and changes, so small problems are less likely to become big ones. Chronic care details.

Transitional Care

The first weeks after a hospital stay can be hard. Many Sandy residents come home from Intermountain Alta View Hospital in Sandy, or from nearby hospitals such as Intermountain Medical Center in Murray, Lone Peak Hospital in Draper, Holy Cross Hospital – Jordan Valley in West Jordan, St. Mark's Hospital, and Riverton Hospital. Our transitional care visits help review discharge instructions and new medications and set up follow-up care, at home. Transitional care details.
