House Call Doctor in Salt Lake City
Salt Lake City has some of the best hospitals in the region, but for an older adult, simply getting to an appointment can be the hardest part. Parking downtown, stairs, winter ice, and long waiting rooms add up. Dvora Healthcare is a mobile physician group, so our doctor and nurse practitioner come to the patient instead. We see people in their own homes and in assisted living, memory care, and independent living communities.
We never see patients in an office. We are also not home health, hospice, or a personal care agency. We are the medical team that comes to you.

From the Avenues to Rose Park
Salt Lake City is a city of very different neighborhoods. We visit older adults in the tree-lined streets of the Avenues and Capitol Hill, the bungalows of Sugar House and Liberty Wells, the east-side homes of the East Bench and Federal Heights, and the west-side neighborhoods of Rose Park, Glendale, and Poplar Grove.
Many families here live in older homes with steep porch steps or basement bedrooms. A house call means your parent doesn't have to manage those stairs, a bus ride on State Street, or a parking garage just to have their blood pressure checked.
How We Care for Salt Lake City Patients
Mobile Primary Care
Your provider gets to know you over time, at your kitchen table. Visits can cover annual wellness checks, new concerns like a cough or a fall, lab orders, and keeping your medication list current. Primary care details.
Palliative Care
For people facing cancer, heart or lung disease, dementia, or another serious illness, palliative care adds a layer of support focused on easing symptoms and planning ahead. You can keep seeing your specialists at the University of Utah or elsewhere while we help at home. Palliative care is not hospice. Palliative care details.
Chronic Care Management
When you manage several conditions at once, details slip through the cracks. Our team keeps in touch between visits to track medications, test results, and appointments, so your care plan stays up to date. Chronic care details.
Transitional Care
Coming home from a big hospital can feel overwhelming. Our transitional care visits help sort through discharge papers, check that new prescriptions make sense with old ones, and arrange the next steps. Transitional care details.
